Your first visit to Northeast Wisconsin Foot & Ankle Associates establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor.

What to Expect for Your First Visit

Our goal is to make your first and any subsequent visits as convenient and anxiety free as possible. Below are some helpful suggestions and hints as to what to expect.

* When should I arrive?

New patients should arrive at least 15 minutes ahead of your scheduled appointment time.

* What can I do to prepare?

New patients can print out and fill out the registration forms ahead of time, to better assure you are seen in a timely manner.

When should I arrive?

If you do not arrive at least 15 minutes prior, we may need to re-schedule your appointment.

* What should I bring with me?
  • Registration paperwork, if you decide to complete it ahead of time.
  • Your insurance card(s)
  • A photo ID, such as drivers license
  • Any co-pay amount associated with your coverage -or- if paying out of pocket, prepare to pay 50% of services rendered
  • List of your medications
  • Any x-rays or medical records from referring or relevant physicians would be appreciated before the appointment
* What should I be concerned about?

Nothing. Our staff and physicians will handle all of your concerns and questions at the time of your visit. Feel free to contact us ahead of time with any questions, so we can make your visit as anxiety free as possible.

*Where do I park?

Parking is available in the parking lot off of Northland Avenue. You will also use the Northland street entrance to our office.
